PATIENTS RESPONSIBILITIES

1. To provide complete and accurate information about his/her health including present condition, past illness, hospitalizations, medications and any other matters that pertain to his/her health.

2.  Provide complete and accurate information including full name and address and other information.

3. To ask questions when he/she does not understand what the doctor or other member of healthcare team tells about diagnosis or treatment.

4. He/she should also inform the doctor if he/she anticipate problems in following the prescribed treatment or considering alternative therapies.

5.  Abide by all hospital rules and regulations

      • Comply with the no-smoking policy

      • Comply with the visitor policies to ensure the rights and comfort of all patients. Be considerable of noise levels, privacy and safety.

      • Weapons are prohibited on premises.

      • Treat hospital staff, other patients and visitors with courtesy and respect.

      • Not to give medication prescribed for him/her to others.

6.  To communicate with the healthcare provider if his/her condition worsens or does not follow the expected course.

7.  To respect that some other patient’s medical condition may be more urgent than yours and accept that your doctor may need to attend them first.

8.  To follow the prescribed treatment plan and carefully comply with the instructions given.

9.  To accept, where applicable, adaptions to the environment to ensure personal privacy and confidentiality of medical records.

You are being directed to CPGRAM website. You would be asked to Register with CPGRAM if not already registered. Registered users can login directly using their CPGRAM user ID and password.

  • 1. Login into CPGRAM
  • 2. In section "Grievance Detail" select "Others/Not Listed/Not Known"
  • 3. In the "Ministry/Department" Drop Down Select ESIC
  • 4. In "Subordinate Department/Office" select the Regional Office
  • 5. Provide details of the complaint

Proceed
